Mandatory Online Participation Forms

9th-12th grade students must fill out each form.

6th-8th grade students must fill out each form EXCEPT the “TAPPS Acknowledgment of Rules” form.

1. To access the required online forms click on the “Electronic Participation Forms” tab.

2. Click on the form name, read and complete the information requested.

3. Only 9th-12th grade student-athletes are required to fill out the "TAPPS Acknowledgment of Rules” form

4. To sign the documents, click inside the signature box and hold your mouse over the box. This will allow you to create an electronic signature. If you make a mistake and need to start over click on the refresh icon next to the signature box.

5. Upon completion of each document and saving, you will have the opportunity to save for your records also.

6. Once all of the electronic forms are completed, move your cursor over to the “Download and Print” tab and click. Here is where you will download and print the Physical form to take to your physician.

7. The Medical History Form must be downloaded and printed to give to physician at the time of the physical. The physical must be dated after May, 2020 to be accepted for the 2020-2021 school year. Once your physician has completed the Physical Form, please scan and email the form to Anna Gober at [email protected] or bring into the athletic office.

**These forms must be completed and on file in the Athletic Training Room before students may participate in SACS Athletics.**"
